The optometrist will want to know about your basic health history and any changes to it since you were last seen. You should be able to answer all of the health questions and know whether there have been any changes to your eyes or your vision since you were seen last.
Benefits Cards and Other Information
If you have vision benefits, you will need to know who covers you and what your account numbers are. Some vision companies will give you a vision benefits card that you can use while others don't. If yours doesn't, make sure you look up your info and have it ready when you arrive at your eye exam appointment. If there have been any changes to your insurance, be sure to let the office personnel know.
Other Items
You might need to bring someone with you if your eyes are dilated and you need a ride home. If you already have contact lenses or glasses, you will need to bring those with you. You will probably be asked to take your contacts out, but the optometrist may want to test you with them first. You will need to have your glasses with you if you want to change out the lenses and keep the frames.
